Bolger Classification of Maxillary Sinus Hypoplasia

  • Bolger et al (1990)1 classified maxillary sinus hypoplasia into one of 3 types:
    • Type 1:
      • Mild maxillary sinus hypoplasia with a normal uncinate process and a patent infundibular tract.
    • Type 2:
      • Mild to significant maxillary sinus hypoplasia with a hypoplastic or aplastic uncinate process.
      • The infundibular passage is ill defined or absent.
      • In these cases, the maxillary sinus is completely opacified by soft tissue density.
    • Type 3:
      • Profound maxillary sinus hypoplasia with an absent uncinate process.
      • In these cases, the sinus is represented only by a shallow cleft in the lateral nasal wall.

Type Features
Type I
  • Mild maxillary sinus hypoplasia with a normal uncinate process and a patent infundibular tract
Type II
  • Mild to significant maxillary sinus hypoplasia with a hypoplastic or aplastic uncinate process
  • The infundibular passage is ill defined or absent
  • The maxillary sinus is usually completely opacified by soft tissue density.
Type III
  • Profound maxillary sinus hypoplasia with an absent uncinate process
  • In these cases, the sinus is represented only by a shallow cleft in the lateral nasal wall
